3-Dimensional Soft Tissue Analysis of Patients Treated with Fixed Palatal Crib Appliance

Backround: The aim of this study was to evaluate the effects of fixed palatal crib appliance (FPCA), which was used to eliminate bad oral habits in mixed dentition, on soft tissue structures regarding lips.
Material&Method: Eight patients (mean age: 9±1.3 years) included in this study who had anterior open bite (overbite: -4 mm) accompanying bad habits such as thumb sucking and tongue thrusting. All patients were treated with a FPCA for a mean duration of 0.5±0.1 year. 3D facial photographs (3dMD) were taken before (T0) and immedietaly after FPCA therapy (T1). The soft tissue analysis including distances of lip superior and lip inferior to TVL, distance of subnasale-lip superior, lip inferior-B’ point and 3D length of lips, was carried out using 3dMD Vultus analysis program. Changes occurring from T0 to T1 were assessed by a paired sample t-test and the relationship between changes in variables was evaluated by Pearson correlation analysis.
Results: Bad habits were eliminated and statistically significant increase were found in overbite with a change of 2.69 mm (p<0.01). The upper lip length were increased significantly (p<0.05), while a slight decrease were observed in lower lip length even there was no statistically significance. There was a positive correlation between change in overbite and upper lip length; negative correlation among change in upper lip length and interlabial gap.
Conclusion: Fixed palatal crib appliance is an effective treatment option for eliminating bad oral habits and correction of anterior open bite by providing positive changes in overbite and upper lip length.
