Özet

The aim of this study is to discuss if postoperative external radiation therapy in non-small cell lung cancer (NSCLC) can improve local control and overal survival or not. Between 1993 and 2004, patients with NSCLC (40 males, 3 females) were treated with postoperative external beam radiation therapy in our department. The median age for whole group was 56 (range: 36-74). There were 3.4 and 36 patients in stage IB. IIB and IIIA respectively. The histopathological diagnosis distribution were as follows: 22 epidermoid ca (51%), 20 adeno ca (47%) and only one anaplastic Ca. Lobectomy was performed in 25 patients (58%), whereas 17 had pneumectomy (39%) and only one wedge resection (2%). External radiotherapy doses ranged betwen 46-60 Gy (median 50 Gy). Radiation portal in all patients were paralell opposed fields involving bronchial stump, ipsilateral hilus and mediastinum. Shielding of medulla spinalis had been performed after 46 Gy. As a result in N1 and N2 NSCLC postoperative radiation therapy is succesfull on local control but no effects have been demonsrated on overall survival. However it has been observed prognostic significance of pozitive surgical margin and residual tumor for local control was demonsrated. Local control has not been demonsrated succesfuly in this group.
