Glioblastoma multiforme is the most frequent primary malignant brain tumor in adults. Standard therapy consists of surgical resection to the extent that is safely feasible, followed by radiotherapy. Major problem after therapy is local failure. In this case report, we present a patient with grade II astrocytoma that recurred as grade IV astrocytoma (Glioblastoma multiforme) 3 months after first resection. Postoperative chemoradiotherapy with temozolomide was administered to the patient. After progression of disease, temozolomide was given to the patient. Imaging studies for differantion of recurrence and brain necrosis was discussed in this article.
