Carbapenem-resistant Klebsiella pneumoniae sepsis in corticosteroid receipt mice: tigecycline or colistin monotherapy versus tigecycline/colistin combination

Özet

Karbapenem-dirençli K. pneumoniae (KRKP) enfeksiyonunun optimal tedavisi hala net değildir. Bu çalışmanın amacı fare sepsis modelinde kolistin, tigesiklin monoterapi ile colistin, tigesiklin combinasyonunun etkinliğini karşılaştırmak, steroid verilmesinin antibiyotik tedavisi üzerine etkisini araştırmaktı. Balb/c fare modelinde OXA-48 üreten KRKP kullanıldı. Antibiyotiklerin in vitro bakterisidal aktiviteleri time-kill eğrisi kullanılarak test edildi. Ayrıca Etest ve broth makrodilüsyon yöntemi ile duyarlılığı (kolistin için minimal inhibisyon konsantrasyonu (MIK) 0.5 mg/l, ve tigesiklin için 0.5 mg/l) saptandı. Fareler kompetent ve kompromize iki ana gruba ayrıldı. Her grup (1) kolistin, (2) tigesiklin monoterapi ve (3) kolistin/tigesiklin kombinasyon tedavileri olarak 3 alt gruba ayrıldı. Metil prednizolon asetat farelere intraperitoneal (ip) verildikten 36 saat sonra 109 bakteri inokülasyonu yapıldı. Üç saat sonra antibiyotikler başlandı ve her 12 saatte bir devam edildi. Farelerden 24 ve 48 saat sonra belirlenenlere ötenazi yapıldı ve kantitatif karaciğer ve akciğer dokularında gram başına koloni sayımı yapıldı. İn vitro çalışmada, kolistin erken dönemde bakterisidal etki gösterdi fakat 24 saat sonra tekrar üreme oldu. Tigesiklin, bakteriostatik etki gösterdi. Kolistin, tigesiklin kombinasyonu da bakterisidal aktiviteyi 8 saatten sonra sürdüremedi. Yirmi dört saat sonra, kompromize ve kompetent gruplarda karaciğer dokusunda, kompetent grubun akciğerin dokusunda hem kolistin, tigesiklin monoterapisi hem de kombinasyon tedavisi kontrole göre bakteri yükünü anlamlı azalttı. Fakat kompromize grubun akciğer dokusunda kolistin, bakteriyel yükü 24 saat sonra kontrole anlamlı azaltmadı. Kolistin, tigesiklin kombinasyon tedavisi monoterapiye göre üstün değildi. Kortikosteroid alan farelerde, kolistin sonrası akciğer dokusunda bakteriyel süpresyon gecikmektedir 

The optimal antimicrobial treatment for carbapenem-resistant K. pneumoniae (CRKP) infection still remains undetermined and less clinical data is available. This study compared the effect of monotherapy of colistin, tigecycline and their combination in sepsis model of mice. The in vitro bactericidal activities of antibiotics were tested using time-kill curves. CRKP OXA-48 strain (with MICs for colistin and tigecycline of 0.5 and 0.5 mg/liter, respectively) was used in Balb/c mice model. The mice were divided into competent and immunocompromised groups. Each group was sub-divided into (1) colistin or (2) tigecycline monotherapy and (3) colistin/tigecycline combination therapy. After three hours of intraperitoneal bacterial inoculation, antimicrobials were administered, and mice were sacrificed at 24 and 48 h. Quantitative bacterial count was assessed in lung and liver as CFU/g.  In-vitro study demonstrated colistin sulfate showed early bactericidal activity, with following re-growth. Tigecycline had bacteriostatic activity and adding of tigecycline to colistin did not generate bacterial killing. In immunocompromised and competent groups of mice at 24 h, bacterial counts in liver samples significantly lowered compared to control, however, there were no statistically differences between monotherapy and combination therapy sub-group. Bacterial count in lung samples of competent group was significantly lesser than control for all three antimicrobial sub-groups at 24 h. Colistin plus tigecycline combination therapy was not superior against colistin or tigecycline monotherapy.
