Early stage Hodgkin’s disease: Experiences of Erciyes University

Özet
Amaç: Hodgkin hastalığı (HH) yeni tanı konulan malign
hastalıkların yaklaşık %1'ini oluşturur. Çalışmada erken
evre HH tedavisi değerlendirildi.
Hastalar ve Yöntem: Takip edilen 119 HH olgusu
içerisinde erken evre 30 olgu retrospektif olarak
değerlendirildi. Erken evre olarak evre IA, IB, ve IIA
kabul edildi.
Bulgular: Erken evre olguların tüm hastalara oranı
30/119 (%25.2) olarak bulundu. On hasta (%8.4) evre IA,
2 hasta (%1.7) evre IB, 18 hasta (%15.1) evre IIA'ydı.
Yirmi iki hasta erkek 8 hasta kadın, ortanca takip süremiz
30 aydı (sınırlar 6-120). Hasta yaşı ortanca olarak 37.5
yıl (sınırlar 16-72) olarak bulundu. Histopatolojik
sınıflamaya göre 11 hasta (%36.7) lenfositten zengin tip,
7 (%23.3) hasta miks sellüler tip ve 12 (%40.0) hasta
nodüler sklerozan tip olarak bulundu. Tüm hastaların 20
si yalnız radyoterapi aldı. İki evre IB ve 7 evre IIA
hastamız COPP, 1 evre IIA hastamız COPP/ABVD
tedavisi aldılar. Yirmi sekiz (%93.3) hastada tam
remisyon elde edildi. Takipte 13 (%43.3) hastada nüks
saptandı ve 2 (%6.6) hasta kaybedildi. Hesaplanan 5
yıllık sağkalım oranı %90.8 iken progresyonsuz sağkalım
oranı %21.3 olarak bulundu.
Sonuç: Erken evre HH tedavisinde kemoterapi ve
radyoterapi kombine olarak kullanılmalıdır.

Abstract
Purpose: Hodgkin's disease (HD) accounts for about 1%
of newly diagnosed malignant diseases annually. In this
study, we evaluated the treatment of early stage HD.
Patients and Method: Thirty early stage cases out of 119
Hodgkin's disease patients were evaluated
retrospectively. Stage IA, IB, and IIA were accepted as
the early stage.
Results: The ratio of early stage to total patients was
30/119 (25.2%). Ten patients were stage IA, 2 patients
stage IB and 18 patients stage IIA. Twenty-two patients
were male, 8 patients female and the median follow up
period was 30 months (range 6-120). Median age of the
patients was 37.5 years (range 16-72). According to
histopathological classifications, 11 patients were
lymphocyte predominant type, 7 patients mixed
cellularity type and 12 patients nodular sclerosing type.
Radiotherapy alone was used in 20 patients. Two patients
in stage IB and 7 patients in stage IIA received COPP,
and 1 patient in stage IIA COPP/ABVD as the alternating
regimen. Complete remission was obtained in 28 patients
(93.3%). Relapse was detected in 13 (43.3%) patients
and 2 died during follow up. The estimated five years
overall survival rate was 90.8% and progression free
survival 21.3%.
Conclusion: Combined modalities with chemotherapy
and radiotherapy should be used in the treatment of early
stage HD.
