Myastenia Gravis (MG) egzersiz ile artan iskelet kası
güçsüzlüğü ile karakterize otoimmun
bir hastalıktır. Tedavisinde, mevcut ilaçlara refrakter hastalarda fayda sağlayacak ve yan etkisi düşük yeni
ilaçlara gereksinim duyulmaktadır. Bu yüzden
yan etkisi düşük bir immunomodülatör ilaç olan teriflunamidin tedavide faydalı olma potansiyelini düşünerek preklinik
bir çalışma yapmayı amaçladık.
Çalışmamızda
aralıklı olarak üç kez asetilkolin reseptör (AChR)
proteininin subkutan enjeksiyonu ile
deneysel MG fare
modeli oluşturduk. Ardından farelerin bir kısmına beş hafta
teriflunamid vererek (10 mg/kg/gün, intraperitoneal) ilaç alan (n:21) ve almayan hasta fare (n:19) grupları oluşturduk.
Hasta grupları ve sağlıklı kontrol grubunu (n:10) birbirleriyle, klinik durum yönünden karşılaştırmanın yanısıra dalak,
lenf nodu ile timus doku
hücrelerinde akım sitometrisi incelemeleri ile T hücre, B hücre ve sitokinler yönünden ve serumlarında ELİSA
yöntemiyle yaptığımız incelemelerle AChR’e karşı antikorlar
yönünden karşılaştırdık.
Çalışmamız, teriflunamidin proinflamatuvar sitokin ve adaptif immunitenin oluşması ile ilgili sitokin üreten T hücrelerinin, antikor
üreten plazma hücrelerinin sayısını azaltmak
ve muhtemelen yaşam süresi kısa plazma hücrelerinin antikor üretmesini sağlayarak AChR’ye karşı oluşan antikor
profilini değiştirmek gibi farklı mekanizmalarla klinik olarak fayda sağladığını göstermiştir. Myastenik
farelerde teriflunamid kullanımı ile
oluşan antikorların IgG tipinden IgM tipine kayma
eğiliminde olduğunu gözlemledik. Bir başka deyişle, teriflunamid IgG oluşumu ile giden sekonder immuniteden, IgM oluşumu ile karakterize primer immuniteye immunolojik
cevapları değiştirmekte, yani sekonder
immunitenin gelişmesini engellemektedir. Bu sonuçların,
teriflunamidin insanlarda MG tedavisinde faydalı bir ilaç olabileceğine işaret
ettiğini düşünmekteyiz.

Myasthenia Gravis (MG) is an autoimmune disease
characterized by skeletal
muscle weakness increased with exercise.
In its treatment, there is a need for new drugs that will
benefit the in refractory patients to the existing
drugs and have low side effects. Therefore, we aimed to conduct
a preclinical study considering the potential of teriflunomide, an immunomodulatory drug with low side
effects, to be beneficial in MG.
In this study,
we created an experimental MG mouse model with three
intermittent subcutaneous injections of acetylcholine receptor (AChR)
protein. Afterwards by giving
teriflunomide (10 mg/kg/day, intraperitoneal) to some of these mice for five
weeks, we formed groups of sick mice
with medication (n:21) and without medication (n:19). We compared the patient
groups and the healthy control
group (n:10) with each other in terms of clinical status, as well as
quantities of T cells, B cells and various cytokines in spleen, lymph node and thymic tissue by means of flow cytometry
investigations and also in terms of
anti AChR antibodies in their serum with ELISA
analysis.
Our study showed that teriflunomide is clinically useful
with various different mechanisms, such as reducing
the number of proinflamatuvar cytokine-producing T cells, the number
of T cells related to the formation
adaptive immunity and the number
of antibody-producing plasma cells, and changing the antibody profile
against AChR possibly by causing short-lived plasma cells to produce antibodies. We observed that antibodies generated in myastenic
mice tend to shift from IgG type to IgM type with its use. In other words, it changes the immunological responses
from secondary immunity with IgG formation to primary immunity
characterized by IgM formation, preventing the development of secondary immunity. We think that these
results indicate that teriflunomide
may be a useful drug in the treatment of MG in
humans.
